What You Need to Know
If your child is fully vaccinated against COVID -19 please fill out this fast online form. You will be asked to upload a photo of the vaccination record, front and back. Once the child’s vaccination status has been approved by the school medical office, your child will not be required to quarantine if he/she is identified as a first level contact of a positive case.
